Intermittent monitoring of LMW heparin for patient's with a high risk of bleeding, renal insufficiency, risk factors during pregnacy, heparin allergy long term LMW use, and for infants and children.

Reference Range
Expected therapeutic range for low molecular weight heparin (measured 4 hours after dosage):
For twice-a-day dosage: 0.4 - 1.1 IU/mL
For once-a-day dosage: 1.0 - 2.0 IU/mL
The patient's result is not accurate if the patient is receiving unfractionated heparin or other anticoagulant.
