Sample Type: Bile duct aspirate, bile duct brushing, hepatobiliary aspirate,  or hepatobiliary brushing.  Preservative ThinPrep vial containing 20 mL PreservCyt or CytoLyt solution.  
If performing cytology before processing specimen, aliquot half of the specimen into another ThinPrep vial. Retain 1 aliquot for cytology and submit the second aliquot for FISH analysis.

Please submit a separate ThinPrep vial for each specimen.

Limitations or Unacceptable Conditions: Submission of the residual specimen (after processing for cytology) may compromise the sensitivity of the test.  A positive FISH result that does not identify the location or type of malignancy result should be correlated with concurrent cytology results.

Methods

Fluorescent in situ hybridization (FISH) analysis of biliary cells utilizing the UroVysion FISH probe kit (UroVysion, Abbott Molecular).  Probes utilized are chromosome centromeres 3, 7, & 17 and the 9p21 locus. The hybridized cells are assessed for gains of chromosomes 3, 7 and 17.

Clinical Utilities

Detection of aneuploidy of chromosomes 3, 7 and 17 in persons suspected of having pancreatic ductal adenocarcinoma, to aid in initial diagnosis of biliary malignancy and subsequent monitoring of tumor recurrence in patients previously diagnosed with biliary malignancy.
