Collection Info:
Submit histology slides, cytology slides, or fixed tissue.
NOTE:
1. A copy of the outside pathological report must be enclosed with the specimen.
2. In cases where certain diagnostic studies, including xray examinations, histologic studies, electrocardiograms, chemical and bacterial
date, etc., have been done elsewhere and are being used as the basis for definitive treatment, either the original or a copy of the report
must accompany the specimen.
3. In cases where histoloic diagnosis has been made elsewhere, written documentation signed by the staff physician including date of
biopsy, laboratory, and pathologist involved with diagnosis, must be provided prior to submission of the specimen.
